Telehealth Billing Services
Virtual visits are simple for patients and complicated for billing. We handle place-of-service codes, telehealth modifiers, multi-state enrollment and changing payer rules so your virtual care gets paid.
Every Payer Has Its Own Telehealth Rules
A telehealth visit can be billed correctly for one payer and denied by the next. Medicare, Medicaid programs and commercial plans each set their own rules for place of service, modifiers, audio-only visits, originating site and which services can be delivered virtually at all — and those rules keep changing.
AccMed RCM keeps a payer-by-payer telehealth rule set for your practice and applies it on every claim before it goes out.
- POS 02 / POS 10 and modifier 95 / 93 applied per payer
- Audio-only and audio-video visits coded correctly
- Licensure and payer enrollment tracked for each state you serve
- Eligibility checks before virtual visits
- Denial follow-up for telehealth-specific denial codes

Which place of service code should telehealth claims use?
For Medicare, POS 10 is used when the patient is at home and POS 02 when the patient is somewhere other than home. Many commercial and Medicaid plans have their own rules, and some still ask for the in-office POS with a telehealth modifier. We follow each payer policy claim by claim.
When do I use modifier 95 or 93?
Modifier 95 usually marks a synchronous audio-video visit and modifier 93 an audio-only visit, but which one a payer requires — or whether it requires one at all — varies. Using the wrong modifier is one of the most common telehealth denial reasons.
Are Medicare telehealth rules still changing?
Yes. Medicare telehealth flexibilities have been extended and changed several times since 2020, especially for non-behavioral visits and patients at home. We track current CMS and payer guidance so your claims match the rules on the date of service.
Can you bill telehealth across state lines?
The provider generally needs to be licensed in the state where the patient is located at the time of the visit, and credentialed with that patient’s plan. We help you track licenses and payer enrollments by state.
Do you bill for telehealth mental health practices?
Yes. Behavioral health is the largest share of telehealth billing we see, including psychotherapy, psychiatric evaluation and medication management visits.
