Overview

How AccMed RCM Handles Medical Coding Services

Coding is where reimbursement is won or lost. A wrong modifier, a missing diagnosis pointer, or an under-specified ICD-10 code can cut payment or trigger a denial that takes weeks to unwind. AccMed RCM’s coding team translates your clinical documentation into accurate, compliant codes that reflect exactly what was done and why.

We code across specialties and stay current with annual code updates, payer-specific rules, and documentation requirements. When documentation does not support a code, we query and guide rather than guess, so your claims are defensible in an audit and your revenue reflects the care you actually delivered.

What’s Included

What Our Medical Coding Services Cover

CPT and HCPCS code assignment

Procedures, visits, and supplies are coded from your documentation with the correct modifiers applied.

ICD-10 diagnosis coding

Diagnoses are coded to the highest specificity the documentation supports, with correct code sequencing.

Modifier review

Modifiers such as 25, 59, and 95 are applied only where documentation and payer rules support them.

Documentation gap feedback

When notes do not support the service billed, you get clear feedback so the same issue does not repeat.

Specialty-specific coding

Coding follows the rules and common denial patterns of your specialty, from surgical bundling to behavioral health time rules.

Code update management

Annual CPT and ICD-10 changes and payer policy updates are applied to your coding workflow as they take effect.

Pre-bill coding review

Coded claims are reviewed before submission to catch bundling errors, mismatched units, and missing codes.

Benefits

Why Practices Choose This Service

Claims that match your documentation

Accurate coding means the claim tells the same story as the chart, which is what payers pay on.

Fewer coding-related denials

Correct codes, modifiers, and diagnosis pointers remove one of the most common denial categories.

Audit defensibility

Compliant coding supported by documentation protects your practice if a payer reviews or recoups payments.

Revenue that reflects your work

Proper specificity and complete code capture stop under-coding from quietly shrinking your collections.

Less burden on your clinicians

Providers document care; our coders handle the coding rules, updates, and payer variations.

Coding Accuracy

Coding Errors We Catch Before Payers Do

Most coding denials come from a short list of repeat mistakes. Catching them before submission is far cheaper than appealing them afterward.

  • Wrong or missing modifiers — 25, 59, X{EPSU}, 26/TC and therapy modifiers.
  • Unspecified ICD-10 codes when the documentation supports a more specific code.
  • Diagnosis not supporting medical necessity under the payer’s policy or LCD.
  • NCCI bundling edits between procedures billed on the same day.
  • E/M level mismatch — levels that do not match documented MDM or time.
  • Outdated codes after annual CPT and ICD-10 updates.

FAQ

Medical Coding Services Questions, Answered

Which code sets do you work with?

We code in CPT, ICD-10-CM, and HCPCS Level II, including modifiers and diagnosis pointers. Coding follows current-year code sets and the payer policies that apply to your specialty.

Can you code for our specialty?

We code for a wide range of specialties, including family medicine, internal medicine, cardiology, orthopedics, pediatrics, behavioral health, and urgent care. During onboarding we review your most common encounters and build specialty-specific coding checks for your account.

What happens if our documentation is incomplete?

We do not guess. The encounter is flagged back to you with a specific note about what is missing, and we share simple documentation guidance so the same gap stops recurring. This protects both your revenue and your audit position.

Is coding included with medical billing or separate?

Both. Coding is part of our full billing service, and it is also available as a standalone service if your billing is handled in-house or by another company.

Do you code from our documentation or do we send codes?

Either. We can code directly from your clinical notes, or review the codes your providers select and flag anything that does not match the documentation.

Will you give our providers feedback on documentation?

Yes. When documentation does not support the code billed or misses details that would support a more specific code, we send clear, practical feedback.

Can you do a one-time coding audit?

Yes. A coding audit of a sample of claims is a good way to check accuracy before outsourcing. See our medical billing audit services.

HIPAA and your data: AccMed RCM works inside HIPAA-compliant workflows, signs a Business Associate Agreement (BAA) before we touch any patient data, and gives each team member only the access their job needs. Please do not send patient details through website forms or plain email — we will set up a secure channel once we start working together.
