Denial Management & Appeals
Find denial patterns, fix root causes, and recover revenue that would otherwise be written off.
How AccMed RCM Handles Denial Management & Appeals
Every denied claim is revenue your practice earned but did not collect, and most practices write off far more than they should. AccMed RCM works every denial systematically: we categorize it, identify whether it is correctable, fix the underlying issue, and resubmit or appeal within the payer’s deadline so the money is recovered instead of abandoned.
Just as important, we track why your claims deny in the first place. Denial trends are reported back to you with the root cause, whether that is an eligibility gap, a coding pattern, a missing authorization, or a documentation habit, so the same denial stops repeating month after month.
What Our Denial Management & Appeals Cover
Denial categorization and triage
Every denial is classified by reason code and worked in priority order, starting with high-value and deadline-sensitive claims.
Corrective resubmission
Correctable denials are fixed and resubmitted with the missing or corrected information the payer requires.
Written appeals
Appeals are prepared with supporting documentation and payer-specific arguments where a denial should be overturned.
Timely filing protection
Appeal and resubmission deadlines are tracked per payer so recoverable claims never expire unworked.
Root-cause trend analysis
Denials are grouped by cause so patterns, like a recurring modifier issue or one payer’s edit, become visible and fixable.
Front-end prevention recommendations
Findings are turned into concrete changes in eligibility checks, coding, or documentation that stop repeat denials.
Denial reporting
Regular reports show denial volume, recovery results, and the top causes affecting your practice.
Why Practices Choose This Service
Recovered revenue
Claims that would have been written off are corrected, appealed, and collected wherever the payer allows.
A denial rate that trends down
Root-cause fixes reduce the total number of denials over time instead of just reworking the same errors.
No missed deadlines
Every appeal window is tracked, so recoverable money is never lost simply because a date passed.
Clear answers about your denials
You see exactly why claims deny and what is being done about each category, in plain language.
Less write-off pressure on staff
Your team stops spending hours on hold and in payer portals; we handle the follow-up end to end.
Most Common Denial Reasons We Work
Every denial carries a reason code (CARC) and often a remark code (RARC). We group denials by cause so the fix sticks.
| Denial type | Typical cause | How we fix and prevent it |
|---|---|---|
| Eligibility / coverage | Inactive coverage, wrong plan or payer | Correct and resubmit; add pre-visit eligibility checks |
| Prior authorization | No or expired authorization | Retro-auth request or appeal; start authorization tracking |
| Coding / modifier | Missing modifier, NCCI edit, wrong code | Corrected claim; coding rule added to the scrubber |
| Medical necessity | Diagnosis does not meet payer policy | Appeal with records; documentation feedback to provider |
| Timely filing | Claim submitted or resubmitted too late | Proof-of-timely-filing appeal; daily submission discipline |
| Duplicate / bundled | Service seen as already paid or included | Review and appeal or adjust; edit checks before submission |

Denial Management & Appeals Questions, Answered
Can you work denials from before we signed up?
In many cases, yes. We start with an A/R and denial review to identify which older denials are still within payer appeal or timely filing limits. Anything still recoverable is worked first, before its deadline closes.
What kinds of denials do you handle?
All common categories: coding and modifier errors, eligibility and coverage issues, missing prior authorizations, duplicate claim flags, documentation requests, bundling edits, and timely filing disputes where proof of original submission exists.
How do appeals work?
We review the denial reason and the claim, gather the supporting documentation, and submit a written appeal or corrected claim through the payer’s process. Each appeal is tracked until the payer issues a decision.
Will we know what caused our denials?
Yes. Denial reporting groups your denials by root cause and shows what changed as a result. That visibility is what turns denial management from rework into prevention.
How long do I have to appeal a denied claim?
Appeal deadlines depend on the payer. Medicare allows 120 days from the initial determination for a redetermination request, while commercial plans often allow 60 to 180 days. We track every deadline so appeals are filed on time.
Is a corrected claim the same as an appeal?
No. A corrected claim fixes an error on the original claim, such as a code or modifier. An appeal asks the payer to reconsider a decision. Using the wrong one can delay payment, so we choose the right path for each denial.
Do you work denials that are already several months old?
Yes, as long as they are still within the payer’s appeal or corrected-claim window. We prioritize by deadline and dollar value.
