Payment Posting & Reconciliation
Every payment, adjustment, and denial posted accurately and reconciled daily.
How AccMed RCM Handles Payment Posting & Reconciliation
Payment posting is where your revenue cycle tells the truth. AccMed RCM posts insurance and patient payments from ERAs, EOBs, and paper checks into your practice management system line by line, matching each payment to the billed charge and the contracted allowed amount. Denials and zero-pay lines are coded with clear reasons rather than buried, so your follow-up team knows exactly what needs work and nothing is written off by default.
Each batch is reconciled against the bank deposit before it closes. Variances between the ERA total, the posted amount, and the actual deposit are investigated the same day, not at month end. Contractual adjustments are checked against your payer fee schedules, secondary claims are triggered as soon as primary posting completes, and patient balances are only moved forward once the insurance portion is fully resolved and verified.
What Our Payment Posting & Reconciliation Cover
ERA and EOB posting
Electronic remittances and paper EOBs are posted at line level with accurate paid, adjusted, and denied amounts.
Denial coding at posting
Every denied or zero-pay line is flagged with a standardized reason so follow-up starts immediately.
Contractual adjustment review
Adjustments are compared with your contracted rates, and underpayments are identified for appeal.
Secondary and tertiary billing triggers
Once primary payment posts, secondary claims are generated and sent without manual delays.
Patient balance transfer
Remaining patient responsibility is moved to patient billing only after insurance resolution is confirmed.
Deposit reconciliation
Posted batches are matched to bank deposits daily, and discrepancies are researched and documented.
Refund and offset handling
Payer recoupments, offsets, and overpayments are recorded accurately and reported to your office.
Why Practices Choose This Service
Accurate A/R balances
Line-level posting keeps your aging report honest, so follow-up works the right accounts.
Faster secondary revenue
Prompt secondary submission shortens the total time a claim stays open.
Underpayments surfaced early
Comparing payments to contracted rates catches payer errors while appeals are still timely.
Clean month-end close
Daily reconciliation means month-end reporting matches the bank, without a backlog of unposted remits.
Transparent denial data
Posting-time denial reasons give you a clear picture of what to fix at the source.
Posting That Finds Money, Not Just Records It
Payment posting is where underpayments, missed denials and secondary claims are either caught or lost. Posting a zero-pay line as a simple adjustment can hide a denial forever; posting a short payment without checking the contract hides an underpayment.
- ERA (835) auto-posting review — exceptions handled manually, not ignored.
- Paper EOB and lockbox posting for payers that still mail.
- Denial and zero-pay lines routed to denial management the same day.
- Contract checks — paid amounts compared with your fee schedules.
- Secondary claims sent after primary payment posts.
- Deposit reconciliation — postings matched to bank deposits.

Payment Posting & Reconciliation Questions, Answered
How quickly are payments posted after the ERA arrives?
ERAs are posted within one to two business days of receipt, and paper EOBs within two business days of scan or delivery. Reconciliation against the deposit happens the same day posting closes.
What happens when a payment does not match the contracted rate?
The variance is flagged during posting and routed to the follow-up team for appeal with the payer contract as support. It is recorded as an underpayment, not quietly absorbed into the contractual adjustment.
Do you post patient payments as well?
Yes. Patient payments from statements, portals, and lockbox deposits can be posted and reconciled along with insurance payments. We follow your office policy for payment plans and partial payments.
How do you handle payer offsets and recoupments?
Offsets are posted against the specific claims the payer identifies, with the recoupment reason documented from the remittance. Your office receives a clear report so cash forecasting stays accurate.
What is the difference between an ERA and an EOB?
An ERA (electronic remittance advice, the 835 file) is the electronic version of the explanation of benefits that payers send with payment. An EOB is the paper or PDF version. Both show what was paid, adjusted or denied for each service line.
How do you catch underpayments?
We compare allowed amounts on each remittance with your contracted fee schedules and flag lines paid below contract for follow-up with the payer.
Do you post patient payments too?
Yes. Copays, card payments, payment-plan installments and patient checks are posted and applied to the correct visits.
