Primary Care Billing Services
Family medicine and internal medicine billing is high volume and full of revenue opportunities that are easy to miss — preventive and problem visits, Medicare wellness visits, G2211, CCM and TCM.
Capture the Revenue Primary Care Already Earns
Primary care practices often under-bill, not over-bill. E/M levels are set lower than the documentation supports, wellness visits are billed as physicals, add-on codes are never used, and chronic care time is never captured. Each visit is a small loss; across a year it is significant.
AccMed RCM reviews primary care visits for correct E/M levels, preventive and wellness rules, and add-on and care management codes, then submits clean claims and follows up on every denial.

Can I bill a preventive visit and a problem visit on the same day?
Yes, when a significant, separately identifiable problem is addressed during a preventive visit, the problem-oriented E/M can be billed with modifier 25 alongside the preventive code. Documentation should clearly separate the two.
What is G2211?
G2211 is a Medicare add-on code for the complexity that comes with being the continuing focal point of a patient’s care. It is added to office E/M visits when that ongoing relationship exists, and it has specific rules about when it cannot be billed. We check eligibility for it on each visit.
What is the difference between the Annual Wellness Visit and a physical?
The Medicare Annual Wellness Visit (G0438 initial, G0439 subsequent) is a prevention-planning visit, not a head-to-toe physical. Routine physicals (99381–99397) are generally not covered by Medicare but are covered by many commercial plans. Billing the wrong one is a common denial.
Do you bill chronic care and transitional care management?
Yes. We bill CCM (such as 99490 and 99439) and TCM (99495 and 99496), which are important revenue sources for primary care when time and contact requirements are met.
How are E/M levels chosen?
Since 2021, office E/M levels are based on medical decision making or total time on the date of service. We review documentation so the level billed matches the record — not too high, and not too low.
