Chronic Care Management Billing
Accurate CCM billing for the care coordination work your team already does.
How AccMed RCM Handles Chronic Care Management Billing
Chronic care management programs generate real clinical value, but the billing rules are specific: qualifying chronic conditions, documented patient consent, a care plan, and time tracked to the minute across the month. AccMed RCM handles the billing side of your CCM program, verifying that documented time and codes align before claims go out, including CPT 99490 for the first twenty minutes of clinical staff time and 99491 for services personally performed by the billing practitioner.
We review CCM claims against Medicare and payer-specific requirements, check that only one practitioner bills per patient per month, and confirm time thresholds are met before submission, including add-on code 99439 for additional clinical staff time where applicable. Denied CCM claims are worked with clear documentation of the cause, and recurring documentation gaps are reported back to your clinical team so the same errors do not repeat month after month.
What Our Chronic Care Management Billing Cover
Code and time verification
CCM time logs are checked against 99490, 99491, and add-on code requirements before claims are submitted.
Consent and care plan checks
Claims are reviewed for documented patient consent and an established care plan in the record.
Duplicate-billing prevention
We confirm only one billing practitioner submits CCM for a patient in a calendar month.
Payer rule review
Medicare and commercial payer CCM policies are applied, including cost-sharing and frequency rules.
Claim scrubbing and submission
CCM claims are scrubbed for diagnosis support and correct place of service before release.
Denial management for CCM
Denied CCM claims are analyzed, corrected, and appealed with documentation when appropriate.
Monthly documentation feedback
Patterns in missing time or consent documentation are summarized for your clinical staff.
Why Practices Choose This Service
Compliant CCM claims
Claims are checked against the program rules before submission, reducing audit exposure and rework.
Captured revenue for real work
Care coordination time that qualifies is billed consistently instead of being left unclaimed.
Fewer time-threshold denials
Minutes are verified against code thresholds, so claims are not submitted a few minutes short.
Clear feedback loop
Clinical teams learn exactly which documentation elements cause problems and how to fix them.
Predictable monthly cycle
CCM billing runs on a set monthly cadence aligned with the calendar-month billing rules.
CCM Codes and Their Requirements

| 99491 | Non-complex CCM personally by the physician or QHP | First 30 minutes |
| 99437 | Each additional 30 minutes by the physician or QHP | Add-on to 99491 |
| 99487 | Complex CCM, moderate or high complexity decision making | First 60 minutes |

Chronic Care Management Billing Questions, Answered
Which CCM codes do you bill?
We commonly bill 99490 for clinical staff time directed by the billing practitioner, 99491 when the practitioner personally performs the service, and 99439 for additional increments where documentation supports it. Code selection always follows the documented time and who performed the work.
What documentation must our team provide?
At minimum: patient consent, a care plan, time logs showing dates and minutes per patient, and the identities of staff involved. We review what you provide before billing and flag anything missing for that month.
Can two providers bill CCM for the same patient?
Generally no. Medicare allows only one practitioner to bill CCM for a patient in a given calendar month. We check for conflicts before submission, which matters most for patients who see multiple practices.
Do commercial payers cover CCM the same way Medicare does?
Coverage is similar in structure but not identical, and policies vary by plan. We verify each payer’s CCM policy and bill according to that plan’s rules rather than assuming Medicare policy applies everywhere.
Can CCM be billed every month?
Yes, CCM is billed per calendar month when the time and other requirements for that month are met.
Does the patient have to consent to CCM?
Yes. Consent must be obtained and documented before CCM services begin, and patients must be told about any cost sharing and their right to stop at any time.
Can clinical staff provide CCM time?
Yes. For 99490, 99439, 99487 and 99489, clinical staff working under the general supervision of the billing practitioner can provide the time. 99491 and 99437 require the physician or other qualified health professional personally.
